pls follow the instructions below when making amendments to ur cca records and cip certs!
CCA Record
1. If there are any additions/deletions/others to be made to ur cca record, pls go to emb/ivle to dl a template form.
2. Fill in the appropriate blanks in the template form and obtain any necessary signatures. Attach it to ur cca record.
3. Uniform grps members are to submit a separate involvement record from ur alma-mater, endorsed by the relevant teacher-in-charge. (contact no. and email of teacher-in-charge to be included)
4. Sign against ur name on the cca record before u hand it in
5. Pass ur CCA record to me by nxt tuesday (12/10/2006).
Involvement Codes:
1. P-S : Participation in intra-college sports event
2. P-NS : Participation in intra-college non-sports event
3. PP : Public Performance
4. E : Organised Enrichment Activity
5. A1-A4 : Applies only to intersch or higher level competition.
A1 - 1st, A2 - 2nd, A3 - 3rd, A4 -5th
6. G,S,B : Gold, Silver Bronze
7. OP : Outstanding Performance
8. R-C : Represented college in inter-sch comp
9. R-S : Represented State in comp
10.R-CN : Represented college in non-comp based event
11.R-SN : Represented state in non-sport based event
12.L-G : Generic leadership position
13.L-S : Specialised leadership position (student conductor/percussion
section leader etc.)
Leadership committee for ad-hoc projects (Camp Unity etc.)
14. External Events: Represented govt or state-recognised organisations

for no. 4 and 13, pls approach teacher-in-charge of the event to submit an email of ur involvement preferably as a list of all students involved (eg. organising committee positions for the project)
CIP CERT
1) If there are errors on ur CIP record, make amendments in RED!
2) Types of amendments:
(A) Wrong CIP hours - indicate the correct no. of hours on the CIP record and attach a
photocopy of the relevant letter, if any
(B) Missing Activity - check if cip activity is found in the list of activities on IVLE/EMB
- IF ACTIVITY IS PRESENT ON THE LIST:
indicate the serial no., name of activity and no. of hours on ur cert
- IF NOT PRESENT:
Refer to list of activities with missing info (check with me
if u cannot find it online on either ivle/emb)
If you find the activity there, pls see miss lim siew hui or ms angel
yang by 5th Oct (Friday) / fill up missing info on the handout and
submit together with cls CIP certs
Attach letter of evidence to ur CIP Cert
(C) Duplication/Spelling error - amend in red
(D) No Certs - 0 CIP hours
3) Submit all ur CIP Certs to me by this Thursday (5th Oct)!
4) Sign on the cls list (the cls list is with me) to indicate that u've checked ur CIP cert and write down the home tel and hp no.
